At Hai Tav Therapy we take client privacy and confidentiality very seriously.

 

This is to inform you what data we are collecting from you and what we intend to do with it.

Full Name, Date of Birth, Address, Email address, Telephone number are essential basic information to identify and to be able to contact you.

Next of kin or an emergency contact, your medical General Practitioner/hospital consultant/any medical professional are also essentials in case of any emergency or if any urgent/important consultation is needed to which you give your consent to be contacted and share only neccessary information.

Session notes are keep for reviewal purposes and are kept in a secure manner out of the reach of others.

It is very unlikely that any of your information or details of our sessions are shared with anyone other than yourself, unless your consent  and approval has been obtained in writing in advance. However, in the unlikely event of an emergency or in case of a legal requirement enforced by law,  we may have to do so, to relevant authorities.  For example, if you or any other person is at risk of any harm, or has been harmed by you or anyone in relation to you.

 

Your data is mainly stored under your online account and access to this site is password protected. Any written notes are kept in files in a secure environment, out of the reach of others. Your phone number(s) and email address may be kept in my mobile phone with your first name and last initial. Only I will access your information.​

​It is required by law that we keep your details, invoices and documents for a minimum of 5/7 years, however session notes, treatment history and medical notes may be kept longer if there is an ongoing treatment for as long as necessary.
